A physician-patient relationship is established only after both you and Nexus Primary Care agree to enter into a professional relationship, all required registration and membership steps, including payment, have been completed, and applicable consent forms have been signed.

Do not use this Website, email, or online contact forms to report urgent medical concerns.

Patients should use our designated secure communication methods for clinical matters whenever possible.

Secure communication options for patients to send protected health information or any sensitive information are either to:

  1. Use your patient portal (including within the Elation Passport app) to send a message, or

  2. Use the Spruce Health app, a HIPAA-compliant communications platform, to text us. While we take reasonable precautions to to protect your communications, if you choose to text the office using standard SMS texting on your mobile device, we cannot ensure communications are secure.

We do not sell your information to any other entity. Your information is only available to a limited number of people who operate on behalf of Nexus Primary Care, and are covered by a BAA agreement.

You can request to have your personal information (or that of a minor for which you are the parent or legal guardian) updated, corrected, or deleted by emailing hello@nexusprimarycarestl.com.
